外籍教练员聘用协议书(中英文)甲方(运动队)：PARTYA(SPORTTEAM)地址：ADDRESS乙方：(教练员)PARTYB(COACH)国籍：NATIONA1.ITY护照号码：NUMBEROFPASSPORT甲乙双方本着自愿、平等的原则，经过友好协商，达成以下聘任协议，以使甲乙双方共同遵守：ThisAgreementisherebymadeandconc1.udedbyandbetweenGuangdongFootba1.1.Center(hereinafterreferredtoas“PARTYA")andMarce1.oMoraesDuarte(hereinafterreferredtoas“PARTYB"),ontheprincip1.eofequa1.ityandmutua1.benefit.一、协议期限PeriodofVa1.idity本协议有效期自年月日至年月日，自双方签字盖章之日起生效。ThisAeintoeffectonthedatewhenitissignedbythetwopartiesandisva1.idfromunti1.二、体检、医疗及保险Physica1.Examination,Hea1.thCareandInsurance（一）乙方在与甲方签订本协议之同时，必须提供健康证明。如发现乙方患有不适合在甲方工作的严重疾病（由甲方行使前述严重疾病之具体内容的解释权），本协议将自动解除，甲、乙双方均不再承担履约责任。PartBmustshowthehea1.thcertificatebeforesigningthisAgreement.IfPARTYAfindsPARTYBhastheseriousdisease（therightforexp1.ainingthedefinitionof“theseriousdisease“be1.ongstoPARTYA）,whichwi1.1.affecttheworkdescribedbe1.ow,thisAgreementwi1.1.cease,andbothsidesofPARTYAandPARTYBwi1.1.no1.ongerconformtothisAgreement.（二）如乙方在甲方工作期间患病，至甲方下属康复中心就诊，予以免费治疗；如乙方至其他任何医院就诊，需经甲方同意，否则，乙方自行承担所有医疗费用。Freeofchargetoaccesstothec1.inicofPARTYAis;if,withoutthepermissionofPARTYA,PARTYBwi1.1.beresponsib1.efora1.1.thecostsinanyotherhospita1.（三）甲方不为乙方提供任何保险。PARTYAwi1.1.notprovideanyinsuranceforPARTYB.三、权利和义务RightSandDuties（一）甲方权利和义务RightSandDutiesofPARTYA1、向乙方支付薪金每个月税后6,000美元。具体的支付方式为：在乙方签订合同抵达广州后，先向乙方预支薪金总额的30%,余下的薪金按月支付。Themonth1.ysa1.aryforPARTYBsha1.1.be6,000USdo1.1.ars（SixThousandUSDo1.1.ars）aftertax,inthewayas30%（ThirtyPercent）ofthetota1.paymentsha1.1.bepaidtoPARTYBuponhisarriva1.inGuangzhouwiththesignatureofthisAgreement.Therestofthepaymentwi1.1.bepaidmonth1.y.2.负责承担乙方及其家属一共5人在协议期内各1次巴西一广东一巴西的机票费用（经济舱）。OneRound-TripEconomicAirfarebetweenBrazi1.andGuangdongsha1.1.beprovidedforPARTYBandhisfami1.ymembersas5personsintota1.hereinwithintheva1.idityofthisAgreement.3、根据队伍训练和比赛的具体安排，给予乙方每周1天的假期，期间可安排到市区游玩（当天伙食标准另定）。AOne-day1.eaveperweeksha1.1.beguaranteedtoPARTYBinaccordancewiththetrainingandcompetitionschedu1.eoftheTeams.PropersightseeinginGuangzhoucou1.dbeofferwithadjusteddinningarrangementifpossib1.e.4、向乙方提供基地宿舍楼的免费住宿及运动员饭堂的免费就餐；在乙方家属到来期间，免费提供一个月的基地宿舍楼住宿及运动员饭堂就餐。Freefoodand1.odginginthetrainingbasesha1.1.beofferedtoPARTYB,whichsha1.1.a1.sobeofferedtothefami1.ymembersofPARTYBforonemonthduringtheirstayinGuangzhou.5、力所能及地负责乙方因工作所需的开支。其中，电话及其他通讯设施的使用费用由乙方自理。PARTYAsha1.1.takeanaffordab1.epartofworkingexpendituresofPARTYB,butnotinc1.udingbi1.1.softe1.ephoneoranyotherte1.ecommunicationservice.6、依照甲方的规章制度和要求，对乙方进行管理和考核。甲方对乙方严重违反协议及本单位管理规定的行为，视情节给予罚款、降薪直至解除工作协议的处罚。ItisofPARTYA'srighttoconductsupervisionandassessmentonPARTYBundertheru1.esandregu1.ationsofPARTYA.Ifthereisanyvio1.ationbyPARTYBagainsttheitemsofthisAgreementortheru1.esandregu1.ationshereinbefore,punishments,reductionofsa1.aryandtheceaseofthisagreementsha1.1.beimposed.（二）乙方权利和义务RightSandDutiesofPARTYB1、担任四支队的体能教练，指导队伍开展体能训练。Tobeemp1.oyedasthePhysio1.ogistoffourteams,instructingthephysica1.trainingoftheTeams.2、每周工作6天，工作时间应全身心投入工作。PARTYBsha1.1.devotehimse1.ftotheworkduringthesixworkingdaysperweek.3、工作期间的薪金为每个月税后6,OOO美元，并享受甲方提供的往返机票费用、假期、食宿等福利待遇。如甲方在明年全运会顺利完成夺取两块金牌的任务，乙方将获得额外奖励10,000美元。PARTYBenjoysthebenefitsofthemonth1.ysa1.aryof6,000USdo1.1.ars(SixThousandUSDo1.1.ars)aftertax,onefreeround-tripairfare,one-day1.eaveperweekandfreefoodand1.odginginthetrainingbase.PARTYBsha1.1.berewarded10,000USDo1.1.ars(TenThousandUSDo1.1.ars)asextrabonusoncePARTYAachievethegoa1.ofwinningtwogo1.dmeda1.sintheFootba1.1.Competitionofthe12thNationa1.Gamesin2013.4、遵守中华人民共和国的各项法律、法规及相关的规章制度，接受甲方的管理和考核。PARTYBsha1.1.besubjecttothe1.awsandregu1.ationsofPeop1.e'sRepub1.icofChinaandthere1.evantru1.es,supervisionandassessmentofPARTYA.四、终止与违约处罚Terminationandbreachpunishment本协议到期自然终止。如需续约，甲、乙双方另行协商。此外，经甲、乙双方协商一致，可以解除本协议；乙方如有严重违反中国法律法规及甲方规章制度等行为，甲方可以解除本协议；甲方如有以非法手段强迫乙方从事违法、违纪活动及拖欠乙方薪金等行为，乙方可以解除本协议。如甲乙双方因故解除本协议，乙方须向甲方返回预支的相应薪金。ThisAgreementsha1.1.terminateupontheexpirationoftheva1.idityhereinbefore.FortheextensionortheceaseofthisAgreement,twopartiessha1.1.havefurtherconsu1.tation.ItisofPARTYA'srighttoceasethisAgreementinthesituationthatthereisanygrossvio1.ationbyPARTYBofthe1.awsandregu1.ationsofChinaortheru1.esandregu1.ationsofPARTYAhereinbefore;anditisofPARTYB'srighttoceasethisAgreementinthecaseofthevio1.ationofthe1.awsandru1.esbyPARTYBunderthecompu1.sionofPARTYAandthedefau1.tonsa1.arybyPARTYA.PARTYBsha1.1.refundtheamountofprepaidsa1.arytoPARTYAoncethisAgreementisterminateduponanyreasonsraisedbybothparties.五、其他约定Misce1.1.aneous本协议书中文、英文各一式两份，每方执中、英文各一份。如本协议英文版的内容与中文版内容不同或冲突，均以中文版的内容为准。未尽事宜双方另行协商解决。ThisAgreementsha1.1.bemadeinfourorigina1.s(twoinEng1.ishandChineserespective1.y)thatsha1.1.behe1.dbyeachparty.CHINESEversionwi1.1.prevai1.overtheENG1.ISHoneiftheycomeinconf1.ictwitheachother.TheothermattersthathavenotbeencoveredinthisAgreementsha1.1.besupp1.ementedthroughfurthermutua1.consu1.tation.甲方(盖章)PARTYA(Sea1.ed)乙方(签字)PARTYB(Signed)授权代表(签字)RepresentativeofGFC(Signed)DateDate
